二、申请人的资格要求：

1.投标供应商应具备《政府采购法》第二十二条规定的条件，提供下列材料：

1）具有独立承担民事责任的能力：投标人须具有独立承担民事责任的能力，提供有效的营业执照

2）有依法缴纳税收和社会保障资金的良好记录：（1）须提供投标截止之日前近三个月内任一个月的纳税证明，依法免税的投标人须提供相应免税证明文件，新成立的投标人按实际的纳税情况提交相关证明；（2）须提供投标截止之日前近三个月内任一个月的缴纳社会保障资金的凭证（专用收据或社会保险缴纳清单），依法不需要缴纳社会保障资金的投标人提供相应免缴证明文件，新成立的投标人按实际的缴纳情况提交相关证明。

3）具有良好的商业信誉和健全的财务会计制度：提供以下两种形式之一的财务状况报告：（1）经审计的最近一年度财务报告（须经会计师事务所审计）；（2）基本开户银行出具的资信证明（如资信证明中未体现为基本户，须附基本户开户证明材料）；

4）履行合同所必须的设备和专业技术能力：投标人须具有履行合同所必需的设备和专业技术能力

5）参加采购活动前3年内，在经营活动中没有重大违法记录：投标人参加本次政府采购活动前三年内（自投标截止时间之日起往前推三年），在经营活动中没有重大违法记录；

合同包1(佛山市妇幼保健院宫腔镜操作系统（加膨宫机）)特定资格要求如下:

(1)法律、行政法规规定的其他条件：①投标人在近三年内（自投标截止时间之日起往前推三年）没有被政府采购监督管理部门禁止参加政府采购活动；②投标人须具有有效的医疗器械生产（或经营）企业许可证或备案凭证和所投标货物的生产（或经营）范围；所投标的货物须具有有效的医疗器械注册证或备案凭证；③投标人所投标的货物为国产产品的，须提供货物制造商有效的医疗器械生产企业许可证或备案凭证；制造商须具有所生产货物的生产范围。(2)本项目允许国产产品或进口产品参与投标；(3)本项目不接受联合体参与投标。

（一）报名登记：

1、符合资格的供应商在获取采购文件时间内，在招标公告附件下载并填写《采购项目报名登记表》（附采购文件费交纳凭证）加盖公章后彩色扫描发送到邮箱（fshh2001@126.com）进行报名登记，发送后请及时联系项目联系人。

2、本项目通过银行转账方式交纳招标文件费用，转账单位名称必须与供应商名称一致，转账时注明“FSHH2131G采购文件费”。交纳凭证截图应附在《采购项目报名登记表》中。招标文件每套售价300元（人民币），售后不退。收款账号如下：
